Serum Total Bile Acids (TBA) increase in patients with acute or chronic hepatitis, hepatic schlerosis, obstetric choleostasis and cancer of the liver. TBA is a useful additional marker to the conventional liver enzymes as it is a sensitive indicator of liver damage.
The Diazyme TBA assay from Alere is used in a large number of laboratories in the UK.

“'The Clinical Chemistry Department at Hereford County Hospital has recently evaluated a new Bile Acid kit available from Alere. The method has been easily automated for use on a Roche Modular system, showing good correlation against our previous 'Enzabile' method. The reagents are supplied in a convenient liquid stable form and initial evaluation studies indicate an on-board stability of at least 30 days and linearity to at least 180umol/l. We propose to introduce the new method into routine use within the next few weeks. This will allow us to perform daily bile acid analysis and significantly improve our turnaround time.'”

“'Our laboratory changed from the Enzabile kit to the CE marked Total Bile Acid Kit. Setting up the method to run on the laboratory's main analyser, Abbott Aeroset, was very straightforward, especially as it is an enzymatic reaction. The reagent is very stable with no noticeable change in results between reagent cartridges (which fit directly on to the Aeroset platform) - calibration being carried out only once every 30 days, or if a new lot number of reagent is started. Accuracy and precision are greatly improved over the previous method and we now have an effective measuring range of 2 - 180umol/l, or up to 500umol/l using the onboard dilution protocol.'”
